    Those black dots appear to be something caked on, not chips in the enamel.

    Enamel doesn’t rust, raw cast iron does. So if there’s rust that means the enamel has chipped off and the raw cast iron underneath is exposed and rusting.

    If it’s a DO, its fine for baking bread if you use parchment paper.

    The rust in the bottom could be two things. Either it’s exposed cast iron in which case you can’t use it safely, or it’s a spot where something else rusted while pressed up against it, and with proper cleaning should be OK. As far as the baked on stuff, I’d start with yellow top easy off in a plastic bag for 24 hours, then a good scrubbing. See where that gets you. Don’t use any abrasive scrubbers.
